What Is FFS?
FFS and facial feminization surgery are interchangeable terms that mean the same thing. The main goal of this procedure is to change the masculine features in the face so that the individual appears more feminine or nonbinary. Testosterone is a hormone that produces characteristic changes that are commonly associated with gender. The FFS procedure can either be performed in one surgery or multiple surgeries depending upon the desired results of the patient.
When FFS is performed, the tissue, muscle, and bone may be manipulated to create a more feminine or nonbinary appearance. Many techniques may be used during the procedure or multiple staged procedures, and many factors are taken into consideration when planning which techniques are appropriate.

How Is Facial Feminization Surgery Performed?
During FFS several areas of the face are worked on, therefore there may be several procedures enveloped in the overall process of creating more feminine features. Each area of the face that has masculine features will be altered to create a more feminine appearance. Here are the most commonly altered areas of the face during a facial feminization surgery.
Forehead
Testosterone in men generally makes the forehead and brow area more prominent than in females which tend to be higher, smoother, and rounded. To make the forehead area appear more feminine, the bone is cut and removed in order to reshape it. Once the bone is reshaped it is put back in place.
Nose
Most commonly, when FFS is performed, rhinoplasty will be one of the procedures during the surgery. Usually, rhinoplasty is performed to reduce the overall size of the nose and the harsh angles that men tend to have.

Cheeks
Women who are assigned female at birth tend to have prominent round cheeks that are set in the middle third of the face. This happens due to there being more fat deposits in the area. To achieve a more feminine appearance, individuals may opt for cheek implants to create a rounded prominent appearance. Sometimes cheek implants are not used, and instead, the cheekbone is fractured and rearranged into a different position. The surgeon may also take fat from other parts of the body such as the stomach or butt, and then insert that fat into the cheek areas.

Chin
Chin reduction is a common procedure performed in facial feminization surgeries. Masculine chins tend to be longer, more square, and sharper than feminine chins. To create a more feminine chin, a horizontal cut is made and a portion of the chin bone is removed. Then the remaining chin bone is reattached.

Lips
Feminine features generally tend to have less space between the base of the nose and the border of the top lip. Lip lifts can be performed during surgery to shorten the distance between the border of the upper lip and the base of the nose to create a more feminine appearance. A lip lift will also add curl to the lip which is also considered a more feminine feature. The lips can also be enlarged by adding filler or implants.
What To Expect After Facial Feminization Surgery
Depending upon what the individual patient wants, the recovery time can vary. Patients should expect pain, swelling, and redness in the face for the first week or so. The areas that are affected more than others can be the cheeks, eyes, lips, and jaw. We recommend that anyone who has gone through FFS to avoid overexerting themselves for at least a month after surgery, with moderate exercise starting around 3-6 months post-op.
Patients should expect some difficulty with chewing or swallowing after FFS. Typically surgeons recommend working with a nutritionist or making an altered diet to ensure the patient is receiving enough nutrients while they are recovering. This is a very important part of recovery, as an undernourished body will not heal as fast of the way it normally would.
